This job is to cover intermediate care bed based discharge work. The QSW will need to be based at St Pancras Hospital and work with a multidiscplinary team to ensure residents of Islington, receive a safe and timely discharge.

St Pancras Hospital operate a P2 approach and Islington residents will receive rehabilitation in an 'in-patient' setting to maximise their independence on discharge.

We are looking for a social worker with advance level skills isn hospital discharge work and would be desirable of the have a knowledge and experience of working with patient with Neuro-rehab needs (brain injury) post stroke etc
