Transitions and Progression to Adulthood Team
Social Work with minimum of 2 years experience. Working with Young Adults aged 18-25 who have a learning disability and are Care Act eligible. The role will involve undertaking Care Act Assessments, completing support plans, completing reviews, safeguarding (all statutory work) and case management/oversight that is required.
Caseload is 20-25.
Flexible working arrangements can be offered but all visits and majority of meetings need to be in person. Need to be in the office around twice a week.
