The Joint Emergency Team (JET), has an opportunity for an experienced Social Worker. As well as being adept at SBA, co-producing care solutions utilising a person centred approach, we are also looking for someone who can utilise their own initiative, has a creative approach to problem solving and thrives in an integrated team that is fast paced. For the right candidate we are offering a contract until the end of March 2025.
JET is a multi- professional team consisting of expert practitioners in Nursing, Occupational Therapy, Physiotherapists, General Practitioners and Social Workers, as well as an extensive supporting staff team. The team provide medical and nursing interventions, urgent social care packages and safeguarding responses for people over the age 18 years, who are registered with a Royal Borough of Greenwich GP and live within the Royal Borough of Greenwich. These referrals are received via our Virtual Ward, 111, GP’s, D2A’s and LAS as well as urgent Social Care referrals from the Front Door Service in Greenwich called the Contact and Assessment Team. JET is dynamic and as such provides a robust service to these crisis referrals, 7 days per week, from 8am to 8:30am. A significant aim of the Team is to ensure that people are supported within their own homes, avoiding the need to go to Hospital.
In addition, we have recently been embedding in an additional pathway, whereby we are responsible for completing Continuing Health Care assessments for those people in the borough who do not have an allocated social care member of staff. It is desirable that the candidate has this experience, however for the right candidate, we will be willing to provide the necessary training to upskill and develop competence in this practice area.
The successful Social Work candidate will need to be flexible and work at least one late shift week, which is 12:30noon to 8:30pm and commit do working one weekend in four (on the weekend that the successful candidate is scheduled to work, you will have Thursday and Friday off that week (there is also an enhanced payment for weekend work attached to this post)) offered training in line with the rest of the team to support their Continued Professional Development, there will be regular supervision in place , as well as having access to a supportive management team to help with case management.
